[Bybit x Sui] Additional Information Required for ByStarter Participation

Note: U.S. persons and residents are not eligible for participation in the sale or distribution of SUI.

If you are a part of the allowlist for Recognition Sale (ByStarter) and have received a notification email to complete a form, please ensure to read the instructions carefully and submit the form provided below.

We require you to fill in this form, as Bybit could not locate a valid Bybit UlD associated with the email address you provided in your previous Sui allowlist application submission.

To resolve this issue, please submit your correct information via the form below no later than Apr 26, 2023, 11:59PM UTC. Failure to submit within the given deadline, can disqualify you from participating in the Recognition Sale.

Things To Take Note When Submitting The Form

(A) The email address provided in Sui's Allowlist registration form will be the only proof that your answers to this form are valid. Therefore:

1. Only fill in the email address you have registered with Sui (as indicated in the red box below).
2. Only submit ONCE. Multiple answers linked to the same email address can disqualify you from participating in the Recognition Sale on SUI ByStarter.
3. Do not share your registered email address with others.

(B) We grant you access to Recognition Sale on SUI ByStarter based on the Bybit UID you provide in this form. Please make sure you submit the correct Bybit account UID that you will use to join the Recognition Sale on SUI ByStarter.

The email address associated with your Bybit account does not have to be the same email that you previously submitted in Sui allowlist application form. This is because we assign the right to participate in the Recognition Sale based on your Bybit UID.

• You will NOT be able to change your UID after submission.
• You can find your Bybit UID after logging in to Bybit, on this page.
